MAQCS Provider Portal Sign Up Information
Create an OKTA account;
1. Go to the OKTA login page (https://okta.loginmt.com/) and click “Don’t have an
account? Sign Up.”
2. Enter your first name, last name, and email address.
3. Open the verification email from OKTA and click Verify Email.
4. Create a password and answer the required security questions.
5. Sign in with your new OKTA username and password.
6. Complete any additional security steps if prompted.
7. After logging in, you will be taken to the Provider Portal to start your application.

Navigating MAQCS, submitting your W-9/EFT, and providers rights and responsibilities;
1. Once you are in the Provider Portal, you will be able to access your facility by selecting “Child Care Services”
2. Select View Dashboard to open the dashboard.
3. Directors, for first-time access to attendance data, must submit the EFT W-9 and Provider Rights and Responsibilities.
4. From the dashboard, go to Childcare Scholarship Provider or Childcare Provider/Childcare Scholarship Provider to start the EFT W-9 submission. This allows you to begin a new EFT W-9 request to view attendance data.
5. Click New Request for a new EFT W-9 submission.
6. Fill in all the required information (marked with a red asterisk).
7. Upload all necessary EFT W-9 files.
8. Click Next to proceed.
9.On the next page, complete additional fields as required and select the required boxes to proceed to the final step.
10.Upload any extra required files.
8. You will also need to submit Provider Rights and Responsibilities. Check all the required acknowledgment boxes and select submit.
